Custom Care Pharmacy is proud to offer the COVID-19 vaccine from Pfizer, through our pharmacy. Vaccines available at select locations.
Our goal is to provide you up-to-date information on how to get your vaccination if you are eligible, and while supplies last. Vaccination appointments are scheduled online only. Appointments cannot be made by calling Custom Care Pharmacy; however, we are accepting walk-ins for the COVID-19 vaccine.

Please complete our online registration form below for your vaccine and bring a valid form of I.D.
Birth Certificates, Social Security Numbers, State ID/DL, Passports are considered Valid Forms of ID.
If you have insurance please bring with you on site.
For Minors: Birth Certificate, Shot records, State ID or Passport is required
Please Note: Submit one registration per person.
Individual registrations are required for each family member.
