I, the undersigned parent/legal guardian of the youth named above, hereby give permission for my child to participate in the event listed above. I understand that this event is organized by the Harris County Sheriff's Office and may involve physical activities or group interaction.

To the best of my knowledge, my child is in good physical condition and does not have any medical conditions that would prevent them from safely participating in this event.

I acknowledge that every effort will be made to ensure a safe environment and that appropriate precautions will be taken to protect all participants. However, I understand that participation in any event involves a certain degree of risk.

By signing below, I hereby release and hold harmless the Harris County Sheriff's Office, its depuites, employees, agents, volunteers, and any other sponsoring organizations or individuals involved in this event from any and all liability, claims, demands, and causes of action that may arise from any injury, illness, or other loss that may occur during or as a result of my child’s participation in this event.
